Transfer of Care Hub (TOCH)

This co-designed, system-wide coordination centre brings together health, social care and wider partners to support timely, person-centred discharges for Surrey Downs patients.

The TOCH supports people to leave hospital, when safe and appropriate to do so, and continue their care and assessment out of hospital. Where practical and safe to do so, initial assessments and discharge plans will take place in your place of residence so you can return home as quickly as possible and continue your recovery with a tailored support plan in place.

When more complex care is required, arrangements to community hospital, interim bedded care, or longer-term placements can also be supported. 

The Transfer of Care Hub brings together a multiagency team - including acute, community health, social care, Xyla and voluntary sector partners - to manage discharges for all patients who require additional support when leaving hospital, including those with complex needs. 

By ensuring patients receive the right care in the right place at the right time, the Hub aims to get patients home more quickly, prevent unnecessary hospital readmissions and promote independence.
