Online Booking
For some services, you can now book your own appointment online at a time and date that suits you.
To do so, you will have received a text message from Surrey Downs Health & Care which includes an invitation to book via a link. Click this link and follow the instructions on the screen.
This service is provided by our supplier Accurx.
Please note - your booking link in valid for 7 days. If this is no longer required or you fail to book within the timeframe you will be discharged back to your GP
Self Referrals
For Urgent Community Response (UCR), MSK, Podiatry, Falls and Dietetics Weight Management services, you can self- refer. Please call our Single Point of Access (SPA) Team on 0208 296 4488.
Heart Failure & Respiratory Services
For Heart Failure & Respiratory services, if you have previously been known to the Service in the last 12 months, you can also Self Refer via the Single Point of Access (SPA) team.
For all other services please ask you GP for a referral.
