Urgent Community Response (UCR)
The Urgent Community Response Service provides an urgent response in the community to Surrey Downs residents. This service is delivered by a local, multidisciplinary team and aims to give patients the most appropriate support in their home, or usual place of residence, to prevent unnecessary hospital admission
Integrated Frailty Service
The SDHCP Whole System Frailty Model delivers a joined-up model to support people when in need and promote independence, reaching individuals across Surrey Downs through multiple touchpoints. Fred’s case study below demonstrates one patient’s journey and how many of the touchpoints played a part in his treatment and continued recovery.
Dementia Coordination Service
The Dementia Coordination Service supports people, families and carers living with dementia by providing personalised advice and signposting to dementia support services in the community. As well as home visits, the team host regular Memory Cafes across Surrey Downs to provide a safe and supportive space for families to meet, seek advice on many matters relating to living with dementia and socialise with people in similar circumstances.
Integrated working with Adult Social Services: Recovery at Home
