Surrey Downs Health and Care is committed to making access to community services as easy and as local as possible. We understand that seeking help can often seem daunting, which is why we’ve worked to streamline the process, giving you the ability to self-refer to a variety of adult services and become more proactive with your, or your family’s health.

We now offer eight different services onto which you can self-refer, either via our Single Point of Access team or via an online app.
