Parkinson's Specialist Nurse and Practitioner Service

Parkinson’s specialist nurses and practitioners are experts in the care of people living with Parkinson’s. Referrals to our service are screened to ensure they meet our criteria and that the individual has a confirmed diagnosis of Parkinson’s made by a neurologist or elderly care consultant. We work closely with your consultant and GP to support you in managing your Parkinson’s. We hold face-to-face clinics in several locations across the Surrey Downs Health and Care community and run telephone clinics to ensure continuity of care. We also do home visits and care home visits for those who are housebound.  

 

We have specialist knowledge of the treatment and management of Parkinson’s, as well as a good understanding of services available in the local area, including health and social services and the voluntary sector. We work closely with other members of the multidisciplinary neuro team and are supported by a skilled team of administrators.

·           Provide information and education about Parkinson’s – and help people to understand it

·           Advise on symptom control

·           Help to manage medication to ensure it is being taken to get the best results

·           Signpost to other services

·           Refer to other health care professionals

·           Provide training for other professionals

·           Support for carers and family

 

Your initial appointment will last up to one hour and you are welcome to bring a family member or friend to support you. Remember, this is your appointment to meet your needs. On the day of your first appointment please bring your completed registration form (sent with your appointment letter) and a list of all your medication. 

If you are unable to attend your appointment please let us know as soon as possible so we can offer this appointment to someone else and find a new appointment for you.
