Surrey Downs Health & Care Partnership Wins Prestigious HSJ Award
Surrey Downs Health and Care Partnership (SDHCP) is delighted to announce that it has won the prestigious HSJ Award 2025 in the newly introduced category of Transforming Care for Older People.
The award recognised SDHCP’s innovative, joined up neighbourhood model, delivered through its formal provider partnership that brings together it’s three GP federations - Dorking Healthcare, GP Health Partners and Surrey Medical Network, St George’s Epsom and St Helier Hospital and Surrey County Council. This long term collaboration has seen partners collectively redesign how care is planned and delivered, guided by the insights and experiences of staff and service users and firmly centred on what matters most to our citizens. This work has led to measurable improvements in patient experience and a reduction in hospital admissions for older adults by over 30%.
