Surrey Downs Health and Care Launches Refreshed Transfer of Care Hub to Support Timely, Person-Centred Discharges

Surrey Downs Health and Care (SDHC) is proud to announce the formal launch of its refreshed Transfer of Care Hub - a co-designed, system-wide coordination centre that brings together health, social care, and wider partners to support timely, person-centred discharges for Surrey Downs patients.

The Hub, recently relaunched through a week-long programme of events, welcomed senior leadership visits and provided learning opportunities to showcase its impact and build awareness of the new model. This celebratory week marks a significant step forward in integrated working, highlighting the improvements made and the benefits for both colleagues and patient care.

The Transfer of Care Hub brings together a multiagency workforce - including acute, community, social care, housing, and voluntary sector partners - to manage discharges for all patients who require additional support when leaving hospital, including those with complex needs. By ensuring patients receive the right care in the right place at the right time, the Hub aims to get patients home more quickly, prevent unnecessary hospital readmissions and promote independence.

Aligned with the NHS ten long-term plan, the Hub streamlines patient flow, focuses on care closer to home and delivers consistent, high-quality support across health and social care systems. At its core is a ‘Home First’ ethos, with the principles of ‘why not home?’ and ‘why not today?’ embedded into every discharge decision.

Looking ahead, the Hub will enable discharge planning to begin at admission, with integrated neighbourhood teams empowered to co-produce discharge plans with patients, families, and carers - focusing on what matters most to each individual.

This new way of working demonstrates SDHC’s continued commitment to delivering fully integrated, person-centred care and a clear focus on neighbourhood working.
