Community Diabetes

Our diabetes teams provide support to adults with type 1 and type 2 diabetes in a number of community locations in the Epsom, Dorking and East Elmbridge areas.

Wherever possible, patients with diabetes are treated by their own GP with support from a Diabetes Specialist Nurse allocated to the practice.

However the specialist team, made up of Diabetes consultants, specialist nurses, dieticians, a podiatrist and psychologist, are based at a number of locations. These hub clinics, which are located at Epsom, Molesey, Leatherhead and Dorking, can provide specialist care for cases that are too complex to be dealt with by a patient's GP practice, but not complex enough to warrant hospital admission.

The teams work with the patient to agree outcomes and then produce a care-management plan, which the patient’s GP practice can then implement.

This approach means that patients with diabetes are receiving specialised care for their condition at the most appropriate level and do not have to make unnecessary trips to acute hospitals.

Our team will help patients to control their diabetes and reduce the risk of complications by providing patients with the skills, knowledge and confidence to self-manage their diabetes effectively.

Additionally, we offer structured education programmes for all patients with diabetes in a variety of community locations.

Who will I see?

Following a clinical referral, a member of the Diabetes Single Point of Access (SPA) administration team will telephone you to arrange a convenient clinic appointment.

During your first appointment, the clinician will carry out a full assessment, with family/carers if appropriate, in order to create an agreed plan of care that best meets the your needs.

If you need to speak with a member of the team, please call SPA on the number below and the message will be forwarded to the appropriate member of staff. The team aim to respond to all messages within one working day.

Accessing this service

We accept referrals from GPs and other health or social care professionals for patients registered with a Surrey Downs CCG GP. This service is open to adults with Diabetes only.
