Your information and what you should know (privacy notice)
Surrey Downs Health and Care (SDHC) is an integrated model of care, supported by a membership of organisations including:
- Epsom and St Helier University Hospitals NHS Trust
- CSH Surrey
- GP Health Partners
- Surrey Medical Network
- Dorking Healthcare
- Surrey County Council (associate member).
The partner organisations will keep personal information about individual’s health, treatment and care. It is important for the partners to keep information about patients and service users so that the best possible care can be provided.
We process personal information for the provision of ‘health and social care treatment to include the management of health and social care systems and services’. This is a ‘Public Task’ as defined in UK law.
Personal information is shared securely between the SDHC partners and health and other social care organisations in order to facilitate the provision of high quality effective healthcare. We only share personal information where it is in the best interests of the individual or where we are under a legal obligation to do so.
